Patient Registration

Before you come to Huntingdon Valley Surgery Center, your surgeon will review with you the details of having your surgery performed at the Center and you will be given detailed instructions. The following forms may be completed prior to your scheduled procedure.

• Anesthesia Evaluation (pdf)
• Consent for Admission and Treatment (pdf)
• Patient's Bill of Rights and Responsibilities (pdf) - This form must be completed and signed in person at the center.
• Workers' Compensation Medical Report Form (pdf)
